PHI includes information that identifies you and relates to your past, present, or future physical or mental health condition, healthcare services, or payment for healthcare services.
We reserve the right to revise this Notice and make updated terms effective for all information we maintain. Updated versions will be posted on our website and made available upon request.
If applicable state law provides greater privacy protections or additional rights regarding your information, we will comply with those applicable state law requirements.

By choosing to communicate with us through these methods, you acknowledge these risks and authorize us to communicate with you in that manner, subject to your right to request confidential communications or opt out of SMS/text messaging as described in our SMS/Text Terms & Conditions.
While we use reasonable safeguards to protect your information, electronic communications may not always be secure and could potentially be accessed by unauthorized individuals. Standard message and data rates may apply depending on your mobile carrier or service plan.
You may opt out of SMS/text messaging communications at any time by following the instructions provided in the communication or by contacting us directly.

The following subsection describes how Seven Bridges Specialty LLC collects, uses, protects, and discloses information collected through our website, online services, patient portal, mobile or desktop applications, email, text messages, and other electronic interactions with us. This subsection applies to personal information collected through online services and does not replace the HIPAA privacy practices described above for Protected Health Information.

Governing law
Any claim relating to the Services shall be governed by the laws of the Commonwealth of Pennsylvania without regard to its conflict of law provisions. You and 7B agree to submit to the personal and exclusive jurisdiction of state and federal courts located in Beaver County, Pennsylvania.
Entire agreement
This Policy, together with the Terms, describe and encompass the entire agreement between us and you and supersede all prior or contemporaneous agreements, representations, warranties, and understandings with respect to the Services, the contents, and materials provided by or through the Services, and the subject matter of this Policy.
